Hearing Back Regarding a Referred Patient
All patient records are protected by the federal confidentiality rules (42 CFR Part 2). Each patient and/or parent will be asked to sign a Disclosure Authorization Form. This will enable our clinicians to release the patient's information to referring physicians, school personnel, and other referral sources. Letters will be sent to referrers only if the patient has signed the disclosure. Please note that it takes a minimum of 2 weeks for the letters to arrive.
Once an evaluation has been completed, the clinician will send letters to referring physicians and school personnel. If you have not heard back from us this may mean that the patient has not agreed to sign the disclosure form.
